Social Services & Discharge Plan Manager

Summary Position Summary The Manager of Social Services and Discharge Planning will maintain primary accountability for the management and delivery of safe and effective patient care and for the day-to-day functions of the Social Services and Discharge Planning team, and other teams as assigned.
The Manager shares the leadership of the service and is responsible for standardizing the operating procedures and coordinating the delivery of social services within the Social Services and Discharge Planning department, and other teams as assigned.
They are directly responsible for the management of care team members, patient outcomes, design, delegation and supervision of workload, and team building.
The manager works closely with the Director of Social Services and Discharge Planning to ensure quality and compliance of clinical documentation.
They are responsible for program sustainability through effective service utilization and caseload management leading to the achievement of the overall clinical and financial goals.
Required Minimum Knowledge, Skills, and Abilities (KSAs) Education:
Master's degree in Social Work, psychology or related field is required.
License/Certifications:
Maine State licensure to practice at the independent level of an LCSW, LCPC, or Ph.
D.
in mental health and substance abuse is preferred.
An LCSW or LCPC is required.
Experience:
5 years of work experience in a health or mental health setting preferred, and 3
years of clinical supervisory and management experience preferred.
Able to teach basic social work principles and applications to the medical setting Proficiency in psychosocial assessment and treatment and considerable knowledge of community resources and agencies and how to use them in resolving complex problems related to health, illness and disability Recommended Skills Clinical Works Diseases And Disorders Financial Planning Health Care Leadership Licensed Clinical Professional Counselor Estimated Salary: $20 to $28 per hour based on qualifications.

The check will bounce and the victim is left responsible.
